Position Description :

The Clinical Resource Coordinator RN to provide case management and discharge planning services, including interacting, supporting, and consulting with physicians, social workers, and other disciplines to coordinate care. Duties also include evaluating severity of illness and intensity of service, as well as assessing patients' post-hospital needs.

Minimum Requirements :

The qualified candidate must be a Pennsylvania licensed RN with a minimum of four years clinical experience. BSN required and Case Management or Discharge Planning experience preferred.
